Scope of Services

Program/Service Structure

  • Population(s) served – Ocean Partnership for Children, Inc. serves youth up to age 21 with behavioral, mental health, substance use, intellectual, and developmental needs.
  • Settings – Care management service settings vary depending on the youth and family’s choice. Services are offered in the youth and family home, in the community, and/or at the offices of Ocean Partnership for Children.
  • Hours and Days of services – Office hours are 9am-5pm Monday – Friday. Services are provided during times that best meet the needs of the youth and family which may occur outside the office hours.
  • Frequency of services – Care managers meet with the youth and families in person optimally twice per month and are in contact with the families weekly.  When a youth and/or family is in crisis, the frequency of services may also increase to meet their needs.
  • Payers and funding sources – OPC care management services are funded by Medicaid on a fee for service basis through the Department of Children and Families Children’s System of Care.
  • Fees – Care management services are provided at no cost to the youth and families.
  • Referral sources – Referrals to OPC care management services are primarily from parents, schools, DCPP, and Juvenile Justice via contacting Perform Care, Children’s System of Care (CSOC) Contracted System Administrator, and Children’s Mobile Response and Stabilization Services (MRSS).
  • The specific services offered – OPC provides care management services directly to eligible youth and families in Ocean County, NJ. Care management services provide linkage and care coordination to community-based services that address identified youth and family needs.

Sharing of Information regarding OPC Services

  • Youth and Families – Each enrolled youth and family is provided with a Welcome Packet and Family Handbook that review scope of services. The care manager also reviews the scope of services with the youth and family as often as needed. The scope of services is discussed at Child Family Team meetings which include persons served.
  • Families/support systems– Enrolled families are provided with a Welcome Packet and Family Handbook that review scope of services.   • Referral sources – Scope of services is available on the OPC website and OPC brochures. Additionally, OPC offers presentations regarding the scope of services to any interested referral sources.
  • Payers and funding sources – Scope of services is available via the contract between OPC and DCF/CSOC as well as the CSOC issued care management handbook.
  • Other relevant stakeholders – Scope of services is available on the OPC website and OPC brochures. Additionally, OPC offers presentations regarding the scope of services to any interested stakeholders such as youth and families served, community-based providers, and CSOC system partners.
  • The general public – Scope of services is available to the general public on the OPC website, Facebook, and Instagram. Additionally, OPC offers presentations regarding the scope of services to any interested parties.
